Please review the following before signing below. I agree that I or my minor child will participate in programs at Inspired Athletx that may include physical training, physical therapy or performance therapy treatments, nutrition advice, and/or sport psychology training and as such may be at risk for injury due to, among other things, physical contact of the body with other persons and/or equipment (including the ground). I specifically waive, give up and release Inspired Athletics LLC (DBA Inspired Athletx) and its employees from liability for any claim for damages which I or my minor child may have for injuries or illness sustained while participating in programs at Inspired Athletx. I understand that during training sessions or while participating in programs at Inspired Athletx that I or my minor child may be photographed and/or recorded on video. I agree to allow said photos or videos to be used for any legitimate purpose by Inspired Athletx, which may include use in promotion and/or advertising materials. I understand that I may specifically deny the use of my image or likeness at any time and for any reason by communicating my wishes to Inspired Athletx staff members.
